To recapitulate: Male/64 - visited a Doctor - blood, urine and sonogram normal - pain in upper abdomen - just below ribs on right side - visible swelling from the back through the right hand side just under the ribs - wants to know whether this can be gas in intestine - the pain is for 3 weeks ...

Put a pal on the swelling and cough or strain to increase the pressure in the abdomen and see if there is an impulse on coughing, whether you can feel the bulge expansile and increase with increase in abdominal pressure; and goes back on removal of straining force.
If this is so, this is most probably a hernia. Although rare to come it is known to occur and is called a lumbar hernia.
Since the contents pull down the mesentery and the organs in the area, the pain is felt in the upper abdominal area as you are having pain in the right upper abdomen just below the ribs.
Another possibilities like lipoma, a large cyst can be confirmed by clinical evaluation and examination by a General Surgeon supported by CT scan and ultrasonography. Please get these done and the diagnosis will be certain and the plan for further management can be planned accordingly.
