Suggest Treatment For Pain On Right Side Of Abdomen

Question: hi i am XXXXXXX ,age 55yrs. for the last one month iam feeling pain in my right side abdomen. the pain arises from the back and goes through right side abdomen(kamar) towards scrotum . i did two sonograohy two ct scanes one mri and two x- rays but the reports of all the tests are normal. i admitted in the hospital for four days where the treatment was only drip and pain killer injections and antibiotic injections. but all in vein. still i am feeling the pain in right abdomen. mainly i feel this pain more in the morning after awkening. at present i am taking the following medicines as per a physician advice.

cap cadbios od
tab pantocid bod
etoshine 90 mg od
tab gabapin 300mg od
tab tryptomer 10 mg hs
till date i have shown to physician ,gestroentrologist ,general surgeon,neurosergon
but still pain is there. pl help. thanks

multiplanar MRI of lumbosacral spine was paerformed using T1 and T2 weighted sequences
CONCLUSION' does not reveal any significant diagnostic abnormality.

no urinary symptoms. urine is passing without any pain or burning

earlier the pain went to scrotum through right abdomen starting from back bone(may be from L3 or L4) but now the pain remains in the right abdomen(kamar) only and that too only in morning after awakening. no pain in thigh or leg.

Thanks for your feedback and the urine report.
Noted your history related to pain in the right abdomen (kamar as you have explained).
This too is in the morning after waking up without any radiation.
Since all the investigations you have mentioned are normal, we can think about the localized problem of the muscles.
Due to the stress of the day these become sore and hence appear tight and hence sore only in the morning on waking up.
As the day passes by the muscles get routinized and hence no stiffness and no pain.
Another possibility is that you might have suffered direct or indirect inadvertent trauma to the muscles and hence got the initial referring pain to the back and to the scrotum which over time vanished and left with local soreness. This may be diagnosed by high resolution ultrasound of the abdominal wall. High clinical suspicion and clinical evaluation and examination by a General Surgeon.

Also watch for any hernia at the painful site.

sir thanks for your reply.
possibility of hernia and appendicyt has already been ruled out by surgeon.
now should i go for high resolution ultra sound of the abdomenal part or contiue the medicines as advised by physician (as mentioned above). not only in the morning but a few times in the whole day also i feel some pain with low intensity.
i am mentioning the results of my earlier usg as below .

usg abdomen and pelvis dt. 3/5/16
impression does not reveal any significant diagnostic abnormality.
adv clinical corelation

usg abdomen dt 5/5/16

impression mild hepatomegaly with grade I fatty liver

ct scan abdomen and pelvis with ct urography.

impression no significant abnormality is seen

Please continue all the medications as already advised.
My opinion is to get the high resolution of the abdominal wall - musculature, whether any muscle problems as compared to other side also. Comparison is very important to get the diagnosis.
It looks all possible investigations have already been done and are reported.
In a few cases repeated tests alone may get a diagnosis. High degree of suspicion alone helps as the routine tests and routinely seen things will be normal and you still have symptoms in spite of medications.
