Brief Answer:
Sits bath will be helpful.

Detailed Answer:
Hi

Thank you for asking HCM

I have gone through your query. Your problem can be most likely prolapsed piles. You can do sits bath with warm saline. This will help to reduce pain and discomfort. You can continue the local cream , antibiotic , laxative- lactulose and supportives. If still bowel movement is not there then you can take 2 bisacodyl tablets or use suppository of it.
Take more plain water. Avoid fried foods. Take vegetable salads and fresh fruits. Taking prunes and prune juice is also good.
If there is no improvement then surgical management will be needed. Hemorrhoidectomy is performed for this. You an consult a proctologist or General surgeon for this.

Let me know if anything not clear.
I am happy to help you.
Thanks.
