Question: I woke up this morning and my groin area was very swollen. I am experiencing pain in my testicles, especially while walking. Can you please help?

- Do you have any associated symptoms like high fever, nausea, vomiting, pain while urinating.

- Is the swelling limited to either one of the testicle or to both?

- Are you sexually active?

- Any presence of engorged veins over the testis?

Since you have pain over the testicles along with sudden appearance of swelling, I would suggest you to consult with a doctor for proper local examination and a scan to look for the underlying cause.

I am extremely sorry for the delay in my response, as I got caught up with my clinical routines,

I have read through the details you have provided,

I will only be able to provide you with some differentials as I am not able to examine it locally:
- epididymo-orchitis- This also appears as sudden inflammation of the scrotal sacs due to infection with some infective organisms. Once properly examined and palpated and the diagnoses is confirmed, a urinalysis is done and then can be started on antibiotic treatment. It will resolve within a week's time.

- Testicular torsion- usually accompanied by unbearable pains and if diagnosed, may require an emergency surgical intervention.

- Abscess formation- also accompanied by pain over the testicles, and if diagnosed should be treated either with medications or by drainage.

- Hydrocoele- accumulation of fluid in the scrotal sac, can be drained and released.

I would suggest you to go for a clinical examination and follow the treatment plan accordingly. Meanwhile, you can start on any oral analgesics for pain relief, and also a scrotal support is advised for you, so as to prevent the recollection of fluid in the sacs.

It's okay, I appreciate your getting back to me.

I went for a clinical examination and was diagnosed epididymo-orchitis. The doctor did a bunch of tests and concluded that it was viral and not bacterial so they did not prescribe me any medicine. I was told that only thing that I can do is to apply ice to the swollen areas.

Do you have any suggestions?

Yes, your doctor is right in not prescribing you any medicine for viral cause of epididymo orchitis. The only available options for you is self care at home.

I would suggest you to do the following:

- keep the area clean and hygienic.
- apply ice packs over the area for 10-15 mins in frequent intervals. This will help in reducing the swelling.
- Also you can do Sitz bath at home, which comprises of soaking the scrotal sacs in a basin of lukewarm water with salt. This also helps in relieving the inflammation.
- If you continue to have pains, you may consider drugs like ibuprofen or naproxen and can also consider serratiopeptidase drugs for reducing the swelling.
- Also I would suggest you to avoid sexual intercourse for the meanwhile.

The viral orchitis is not of any major concern. It will resolve in 1-2 weeks time.

Thank you for the detailed answer, I really appreciate it.

I've been applying ice packs for 15-20 minute intervals and I also tried the Sitz bath, both have really helped. Thanks! I'm no longer having any pain and the swelling has improved immensely.

One question though - because of the swelling, I'm still not able to wash the tip of my penis like I normally would. Do you have any suggestions?

Glad to know that your swelling has improved immensely,

It is normal to have associated swelling over the penile region, and the associated inability too retract the foreskin. It normally takes a couple of weeks to resolve completely.

Meanwhile, I would suggest you to do the following:

- Immerse the penile region along with the scrotal sacs while doing sitz bath. Since it contains lukewarm water and salt, this will act as a cleansing agent and will clean the tip of penis.

- Do not try to retract the foreskin while its inflammed, as it may cause any tears beneath.

- Also the undergarments should be washed in warm water with mild detergents so that the bacteria are removed. It is also better to wear cotton undergarments to allow healing to take place.

- Just keeping the area clean and dry is enough to prevent any further infection.
