Regularly, I come across many patients who have groin lymph node enlargement. In most of these patients the lymph node enlargement is benign and due to any infection in the area drained by the lymph nodes. The groin lymph nodes drain the entire lower limb on that side and groin. Since you have 4 swollen lymphnodes, it is possible that you have some infection process going on. One lymphnode can be swollen more than the others. The pain in the lymphnodes usually occurs XXXXXXX to infection and the lymphnodes enlargement is a protective mechanism adapted by your body immunity.
Most infections are identifiable and sometimes it can be treated with a short course of antibiotics. The fact that your thigh is larger on the affected side shows that you might have developed cellulitits. This requires to be treated with medications to prevent complications. Please consult your treating doctors at the earliest.
If enlarged lymphnodes do not respond to oral antibiotics then a lymphnode FNAC or biopsy might be indicated to confirm the cause. FNAC or biopsy nearly always gives a diagnosis on which the treatment can be based.

I realize that you do not have fever and acute swelling of your limb and therefore cellulitis might not be the correct diagnosis. However since you have written in about significant weight reduction measures adopted by you and possible loosening of knee replacement hardware, it is to be confirmed if this is responsible for your inguinal lymphnodes. It is possible that there is reactive lymphadenopathy due to low grade infection or in response to loosening of the metallic hardware in your affected limb. It is not necessary for outward signs or elevated white blood cells to be present should there be a low grade infection.
The above can be confirmed by doing a detailed evaluation of your affected limb.
