Question: I have painful urination and my bladder doesn't seem to empty, I have frequent panic urination with very little urine coming out. This has been coming and going for a few months. When I go to bed this seems to stop and the pain seems to leave until the next morning and then is starts again. I have accidents when I can't make it to the bathroom in time.

Brief Answer:
prostate is the most common cause

Detailed Answer:
HI, thanks for using healthcare magic

There are different causes for your symptoms but with your sex and age, enlargement of the prostate would be the most common or likely cause.

Enlarged prostate can causes painful urination, frequent urination, incomplete urination, incontinence. Prostate enlargement is most usually benign (non cancer)
If your prostate has been assessed with the last few months both by physical examination and blood tests and this is not likely , then other causes should be considered.

The other common causes would be: (1) infection of the urinary tract
(2)presence of mass in some aspect of the urinary tract
(3)stone in the urinary tract
(4)neurological- related to abnormal function of nerves in the area

Full evaluation would require a physical examination, blood tests (PSA, FBC- full blood count, ESR- this checks for inflammation in the body) and possible imaging of the urinary tract by ultrasound.

Treatment would depend on the cause found

I hope this helps, feel free to ask any other questions
