By seeing the picture you had sent it appears like you are suffering from scrotal varicocele.
To know the grade doppler study may be needed. I would suggest you to consult a surgeon and get examined.
The treatment for varicocele is done only if person is having pain or dragging sensation or reduced sperm count.
I would suggest you to get your sperm count also if you dont have a child yet. If you have then no need for this.
The treatment for varicocele is surgery . Drugs don't have much effect.

But me paint veins in the bottom. Can I try drugs? I can't use the underwear.

Yes I have seen the picture you have sent. In Varicocele veins may be seen above the scrotum or at the bottom also. Commonly we see above the scrotum where cord is there near root of scrotum . It may be seen at the bottom also.
Drugs don't have very much effect in the treatment. So always surgery is adviced. Drugs like calcium dobusolate may be taken with no proved efficiency. Painkillers for pain management.
If you are unable to use underwear then I suggest you should discuss regarding going for surgery with you doctor(general surgeon).
Apart from surgery you may try percutaneous embolisation. This have good effectiveness like surgery.
