Suggest Treatment For Panic Disorder

Question: Hello sir, I had first panic attack before 3 years & I was prescribed duzela 20 once every day, also I was suggested to keep lonazep .50 md to keep with me for any similar attach. I started meditation & was cured almost fully because of meditation. I did not suffer any other attack for last 3 years but for last 2 weeks again I had 2 such problem and had to take lonazep .50 . It happened during my flight from London to Dubai. And again I faced it in my car after 3 days. Can you kindly suggest any permanent cure, also any other medicine in case of emergency. I read your query and understand your concerns.

The management of panic disorder can be done by using two approaches and one is already used in your case and fortunately found good response barring last two weeks.

The second is behavioral or psychotherapeutic approach which needs time but leads to more long lasting response than the medication.

Having said this following can be done
1. Use lonazep 0.5 mg regularly for two weeks and than decrease the dose gradully and meantime increase the dose of duzela to 30 mg and keep it there. Our purpose for two week lonazep is to give cover for increased dose of duzela and once it starts showing appropriate concentration in body we need to withdraw lonazep.

The second option is to search as psychologist and get CBT for panic attacks. I know this is difficult as very few clinical psychologists are available but if you get one this is the best thing to do. As I said above the psychotherapy helps in permanently curing the condition and on many occasions person can stop taking medications.

I thank you for the quick response. My family doctor has increased the duzela20 from one to two tablets daily for a month. Reg Lonazep, he advised to take only when I get the attack. So what is your opinion in the matter. Kindly suggest. Regards. XXXX

Thanks for reverting back to me.

As suggested clonazepam can be taken on need basis but considering two back to back attacks I feel a cover of 15 days can be given to protect from any duch episodes.

Having said this I feel 40 mg of duloxetine is good enough and I hope by the end of two weeks you will get better in regard to risk for further episodes.
