Question: my 25 year old daughter was wiping and saw a tiny dark bug (about the size of grain of rice/black)when she wiped her anus. this has happened twice this morning. Her anus is inflamed and she says it itches. I do not see any bites

I appreciate your concern

Based on your description, it appears to be parasitic infestation or an insect bite. She should clean the area with antiseptic like povidone diluted in water. Permethrin cream can be applied externally along the skin. A bath with lukewarm water should preferably be taken and the current clothes should be changed including any undergarments. Local calamine lotion can also be applied to control associated itching and inflammation. Antihistamines like levocetirizine can be taken if the local treatment does not give adequate relief. Stool for routine examination and for ova , parasite should be done. Treatment can be started without waiting for the test results by administering mebendazole single 500 mg oral dose for deworming, in consultation with your doctor.
