Detailed Answer:
Thanks for your query at HCM.
I understand your concern and situation.
Yes, your test is showing that you are HIV positive. Your blood has HIV RNA levels. Recent guidelines suggest if CD4 count is below 350 you have to start antiretroviral treatment. Your CD4 count is below 200. It indicates that you must immediately start treatment for lifetime. As you are having advanced HIV stage 4 which is now full blown AIDS you are at risk of various opportunistic infections such as tuberculosis, parasitic infection of intestines such as cryptosporidium, cyclospora, isospora, salmonellosis, fungal infections of skin, molluscum contagiosum, etc.
It suggest get your stool test done for microscopy, special staining like modified acid fast staining, trichome Weber staining to find out parasites infection causing trouble. Based on reports you can start treatment with nitazoxanide, metrogyl, etc. Now for your skin infection, you have to describe lesions it could be viral infection like molluscum, etc. You can get skin scrapping test done to find the cause which you can get from reputed laboratory. Treatment will be based on test results.
Do you have cough? If yes, get sputum examination for acid fast bacilli and X Ray. You must also get tested for other sexually related diseases. Get HepatitisB vaccination. You have to star first-line ART should consist of two nucleoside reverse-transcriptase inhibitors (NRTIs) plus a non-nucleoside reverse-transcriptase inhibitor (NNRTI) like tenofovir, lamivudine and efavirenz fixed dose combination.
Consult a doctor and start treatment to prevent further infections. Take precautions while sexual intercourse.

Detailed Answer:
Hi
I welcome your query.
You must know if CD4 count fall and viral load increases this leads to severity in disease and results in complications. But don't worry there are anti retroviral drugs available with compliance things will improve. I suggest you that you can always ask direct questions on HCM as I stay far from your place. I suggest visit ICTC center in your area they will guide you reliable government ART centers available in your area for treatment. I suggest start your treatment immediately that will improve your immunity.Take plenty of fluids, eat protein rich balanced diet with multivitamins and calcium supplements.Avoid unpasteurized dairy products, raw eggs and raw seafood such as oysters, sushi or sashimi. Cook meat until it's well-done or until there's no trace of pink color.Take vaccination for flu to prevent infections such as pneumonia and the flu. Make sure the vaccines don't contain live viruses, which can be dangerous for people with weakened immune systems so take killed vaccines or recombinant only. Avoid certain animals may carry parasites that can cause infections like Cat feces can cause toxoplasmosis, reptiles can carry salmonella, and birds can carry the fungus cryptococcus or histoplasmosis. Eat fish oil and whey protein. Tell your sexual partners about your disease and take precautions.
Take care.
Don't worry.
I have uploaded test reports of endoscopy, colonoscope, biopsys, stool test, vb12 serum and Mantoux for skin. The doctor I consulted suggested me to take 1 tablet of Forstavir-3, Please please please provide me medication for chronic diareah. Severe current issue is loose stool more than 10 times a day. Even the gastroentrologist and ID doctor have not given medication for stopping watery stools. I beg u for medication for chronic diareah to gain weight and confidence.
Take nitazoxanide and plenty of fluids.
Detailed Answer:
Hi dear. Don't worry so much. You need to relax.
I suggest drink more clear liquids, avoid caffeinated products, refrain from milk products
eat sources of soluble fiber, such as oatmeal
avoid greasy and spicy foods. You can start food product based on bovine colostrum powder. Take Nitazoxanide 1000 mg twice daily with food for 14 days or until diarrhea resolves. Anti retroviral treatment will also help to reduce diarrhoea. Take ORS after diarrhoea. You can take loperamide also if too severe diarrhoea. Eat potatoes, rice, bananas, soup and boiled vegetables and salty foods. Take a probiotic satchet also.Forstavir 3 is Efavirenz/ emtricitabine/ tenofovir anti retroviral drugs. It will help you gradually. Get your stool test for routine microscopy examination for parasites and culture sensitivity test for aerobic and anaerobic bacteria. In the meantime also get evaluated by a psychiatrist. It will help you immensely.
Hope I was helpful.
