Suggest Treatment For Penis Pain Due To Penile Fracture

Question: Hello, About a month ago, my wife of 20 years and I were engaged in a passionate time of love making in missionary position. I was on top. I remember that my left knee was slipping off the edge of the mattress and I lifted her hips as I attempted to move us both toward the center of the bed. Anyway, in the midst of this "maneuver" I heard my penis "snap" and felt an immediate dull pain. We finished to orgasm without any trouble, and I asked if she heard the sound. She had heard it as well. Within a few minutes, I was able to achieve another erection without pain and helped her climax externally. Over the new few days, I noticed a dull pain in my penis through the day but no pain during erections, urination or anything I would normally do with it. So about a week ago, she and I had intercourse again and deep penetration was initially uncomfortable, then painful for me. We were still able to make love, but the pain near the base and top of my penis was fairly strong and distracting. After that time, I noticed an increase in the frequency of pain. While I was urinating a few days ago, I noticed a lump at the base of my penis, right in the center of the tissue, which was painful to touch and still is. This was the first moment I had felt it. While showering today, I was washing myself and got an erection, which was painful unless I supported it with my hand at the base. I also felt a tension in my penis - a painful "tugging" inside when I did not support the erection with my hand. The erection went away quickly. I can tell that something is wrong. I'm just not sure how severe this is, or if I need to see someone about it. There has been no bleeding, bruising, or change in appearance whatsoever. Just the painful erections and a dull ache throughout the day occasionally. Thank you.

Brief Answer: Small Penile fracture? Detailed Answer: Hi, Almost certainly you have what we term 'penile fracture'. Most likely your erect penis was thrust forcibly against your partner when you were manipulating for a more convenient position. Sometimes, the fracture is a massive one where all the congealed blood which is present at the time of a normal erection in the tubular engorged structures called corporae cavernosae leaks out through an opening in the lining of the corpoare and causes immediate loss of erection and pain with patients and partners describing a 'popping' sound. You must have had a much smaller defect in the lining of the corporare due to which you were able to complete your act at that time, but there must have been persistent slow ooze and that has caused your present level of discomfort and even the induration in your phallus. You certainly need to see a urologist who specializes in these matters, and get some basic investigations done. These could include an ultrasound examination of your penis in both flaccid and erect states, and if required also 'cavernosogram' and retrograde urethrogram, following which we will be in a better position to advise you. For the moment, I would suggest that you do not attempt intercourse till things have become clearer. I do understand that you might not want treatment for this problem and see if things get better on their own, but we cannot be sure that will happen, and secondly, even a natural healing could cause some sequelae like a curvature of penis or persistent pain during intercourse which I am sure you will not find acceptable especially since you are still fairly young. Hello, Thank you for your thoughtful reply. I am lookikg for a male health specialist in our proximity right away. Since I posted this question, today I noticed something I have never experienced before while gaining an erection. The bottom half of my penis swelled as usual, but the middle of it was indented from both sides, and the top half swelled more slowly. After a moment, the "hourglass" filled in and I had a solid erection as usual, although a bit uncomfortable. I was shocked at this event, because it was abnormal for me and I'd never heard of it before. I felt some degree of pressure and dull pain during this process, but just slight discomfort as I became fully erect. Should I try and avoid erections and all sexual activity until I can see a specialist? Measured oral or manual sex with my wife as well? I've read that bloodflow to the chambers will help to remove damaged tissue / help maintain flexibility. However, I will rely on whatever you recommend. I ordered some jockstrap underwear for more support. I've also ordered a topical and nutritional protocol from England which has proven to be very beneficial in restoring tissue and function for others with similar injury. Hopefully I will be able to see a local specialist as early as next week. Thanks again foe your valued perspective and guidance. This has really had an impact on me as a man!

Brief Answer: Try to avoid erections Detailed Answer: Hi again. You are welcome anytime. Glad to have been of help. I certainly would recommend that you try and avoid erections attained by any means till your problem is resolved to your satisfaction. Apparently people have tried giving men female hormones like Estrogens to reduce libido and erections. While I guess that might be going far in your case, certainly letting the corporae heal will do no harm. I have never heard of blood flow improving damaged tissue of a suspected penile tear. It could help in a condition we call priapism, but not in your case (priapism is where a man has a prolonged, painful erection even when he is not stimulated). So please do not attempt it till you get definite advice from a urologist. Local support should certainly be helpful. I doubt if any nutrition will help though, and I hope you are not shelling out too much for it. Please let me know once your investigations are completed, and if you have any doubts I will be happy to clarify.
