I can understand your concern for the unusual appearance of lip in your grandson.
I have gone through your complete query and noted your concern. After diligently analyzing the picture and correlating with the details let me guide you that it appears to be simple Lip licking dermatitis.
To tell you elaborately it can be due to any minor friction or abrasion over the lower lip area. As the lip area is quite soft so even a single time licking over the affected arwa makes the area prone for further damage in children.
I can appreciate that you have observed that the child is not over biting. But I would like to guide you that mere licking or touching the lower lip with tongue could cause it. The saliva in such a sensitive skin could irritate the skin. I agree with your treating dermatologist but jot by biting rather simple licking or touching only.
I suggest you to meet your dermatologist again and get proper treatment for this. There is no point of MRSA in the same so don't worry about the same.
