Suggest Treatment For Persistent UTI And Diarrhea

Question: I saw my family Dr. because of a UTI. It has been a persistant problem. I was also having persitent diareaha. He put me on Ampicillin for 7 days for the UTI. When the stool sample came back with a diagnosis of C. diff, he added Flagyl. The info with the Ampicillin says it may cause C diff. Do I need to check with him about taking both meds?

Thanks for query .Based on the facts that you posted that you have been taking multiple antibiotics for UTI since 2 years and recently got diarrhea after taking Ampicillin .

Diarrhea that you have is known side effect of Ampicillin .

I would suggest you to get urine culture done and take appropriate antibiotic as per culture report .

As regards your problem of persistent UTI I would like to make few comments about the cause and treatment of your UTI in detail though you have not asked for in your query .

The repeated episodes of UTI that you are getting is mostly due what is termed as Senile Urethral Stenosis .

In females as the age advances there is generalized atrophy of Genito Urinary organs which also involves urethra leading to narrowing of urethral passage (Urethral Stenosis),This stenosed urethra causes incomplete emptying of bladder while passing urine leading to adequate quantity of urine to remain in bladder what is called as Post Void Residual Urine (PVR) this residual urine is the source of getting repeated episodes of Urine ,

You need to consult qualified Urologist and get following basic tests done to establish diagnosis .
1) Urine routine and urine culture
2) Ultrasound Scanning of abdomen with specific evaluation of Post Residual Urine volume .
3) Endoscopic examination of bladder

He may do urethral calibration(Urethral Dilatation ) at the same sitting .
This will help to widen the urethral passage and thereby effective and complete emptying of bladder with negligible residual urine which is the primary source of UTI..However you will need to learn to do Clean self Intermittent Catheterization (CSIC) every day at home to empty bladder for 3-4 months .This will help to resolve the problem for forever .and you may not need to take antibiotics . .

I routinely follow this regime in my elderly female patients who come to me in my practice for last 45 years and getting encouraging results .

You can discuss this regime with your Urologist . I am sure you will be benefited with this treatments .

I have had the urology eval including
1) Urine routine and urine culture
2) Ultrasound Scanning of abdomen with specific evaluation of Post Residual Urine volume .
3) Endoscopic examination of bladder
and I am emptying my bladder. The urologist put me on 4 months of macrobid. That solved the problem through the summer April - Aug. but now I've had 2 sessions of UTI. The first one, the urologist put me on 10 days of macrobid. The second UTI started 5 days later. Since I couldn't get to the urologist, I saw the family Dr.

Please also address the aspect of my question about the antibiotics and C diff. I'm also taking probiotics 3 times each day.

Stop taking Ampicllin and take Ofloxacin with Ornidazole

Thanks for follow up Stop taking Amicillin ,Instead you can take combination of Ofloxacin and Ornidazole twice daily for 5 days as Flagyl has side effect of nausea and stomach upset ..

For UTI take appropriate antibiotic as per culture report .
