Suggest Treatment For Persistent Belching And Swollen Ankles

Question: Hello, Last night I noticed my ankles were swollen, later I had annoying belching the rest of the night. I had a beef sandwich to eat for dinner. This morning, the belching is still there along with shortness of breath and an odd discomfort under my left breast. I took my blood pressure it was 165/86 heart rate was 92. I do take Lozartin 25 mg. once a day for blood pressure. I am 70 years old. My question, am I on the verge of a heart attack? Thank you.

I passed carefully through your concern and would like to explain that your complaints of belching and discomfort seem more likely to be related to a gastrointestinal disorder such as hiatal hernia, gastroesophageal reflux.
in this regard, it is advisable to avoid spicy fatty foods and try any gastric anti-secretor drugs such as omeprazole, pantoprazole, etc.

From the other side, ankle swelling needs to be further explored and differentiate between:

- heart failure,
- local venous insufficiency,
- a kidney disorder

For such purposes a careful physical exam and some medical tests are recommended:

- resting ECG,
- cardiac ultrasound,
- BUN & creatinine,
- blood electrolytes level,
- plasma protein level,
- chest X ray study

If the chest discomfort persists an ECG and cardiac ultrasound should be anticipated to exclude any clinical scenario of cardiac ischemia. In such case asking medical assistance to the nearest ER service would be a rational approach.
