Brief Answer:
As detailed below.

Detailed Answer:
Hi.
Thanks for your query about your husband.
To recapitulate: Male/75 - has dementia - complaining of headache - fell a few months ago - bump on forehead that never went away - moved to bridge of nose with swelling of eyes and nose this morning on getting up - refuses to go to a Doctor - wants to know if this is serious and need to tell him something so that he may let you take him to a Doctor....
The most important point to tell him would be presence of headache along with shifting of the lump - swelling of the eye and nose can cause more problems.

The cause as asked by you is:
The special arrangement of the skin of scalp over the bone of the skull makes different pockets. One of these is a tight one just over the bone which is potential space and the lower attachment is up to an area as limited by the now-shifted swelling.
This is usually a hematoma or a seroma and not much a serious problem.
Yes, surely should have a clinical evaluation and an actual examination of the nervous system as there was a head injury and that he has headache. A CT scan or MRI may be required if thought so by the examining Doctor.

I hope this answers you query and helps you understand what exactly has possibly happened.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
