I feel my query should have been answered by a Physician atleast and not an Ophthalmologists
Brief Answer:
I am not ophthalmologist, I am pulmonologist.
Detailed Answer:
Thanks for your follow up question on HCM.
I am not ophthalmologist. I am pulmonologist.
And I deal with patients who are having complaints of cough. I deal with many patients of bronchitis daily.
You are smoker and chest x ray suggestive of chronic bronchitis. You are also having diabetes. So all these factors favour bronchitis in you.
First line treatment for bronchitis is inhaled treatment consists of inhaled bronchodilators and inhaled corticosteroid.
You are not taking any of these.
This is the probable reason for not improvement.
Smoking should also be stopped as soon as possible.
Hope I have solved your query.

Most common cause for bronchitis is viral infection. And it can also cause nasal congestion.
So both share common etiological agent.
Inhaled corticosteroid (ICS) like budesonide and fluticasone are commercially available.
Budesonide plain is available as trade name of budecort. You should take it for atleast 1 month.
