I need to get information of your reports of blood and other investigations to guide you for your dizzyness and fatigue.
I suggest you the following things
1.please check your fasting and post meal blood sugar along with HBA1C.Fasting must remain between 100 and 110,Post meal below 180 and HBA!C must remain below 7%.You should inform me about your medications so i may suggest some addition or alteration if needed.
2.Please get your lipid profile and kidney functions checked.
3.Get your Electrocardiogram done,
4.please let me know about your menstrual history,Habits and level of blood pressure
Meanwhile you should take multivitamin,mineral and antioxidant preparation along with your regular medicines,

I understand that your sugar is under good control.
I would request you to go for further work up for your stomach problem.Taking Prevacid and Reglan for 10 years is too lengthy a period.I suggest you to get Ultra sound of upper abdomen and if needed Upper GI endoscopy.
Please get your exercise cardiac stress test done.sometimes extreme fatigue may be symptom of heart disease in diabetic.
please check Xray of Cervical spine to rule out spondylosis as cause of your dizziness.
Please give up smoking or alcohol if at all you are consuming,
