Suggest Treatment For Persistent Dryness Of Skin On Penis Glans

default
Posted on Fri, 12 Jun 2015
Question: Hi, about 5 months ago, I put on some cargo shorts without underwear for the day and when I took them off, their was a small pink spot on the top left side of my glans about a 1/2 inch wide that was assymetrical. There was also what seemed to be flaky dry skin on top of the spot. My first thought was that it was a mild zipper scrape or just irritation from the cargo shorts fabric without a barrier. I've gained 15 pounds or so over the last 8 months and never really changed into large sized clothing.

To make matters worse, I took a hot shower as I typically do, and basically peeled off the top layer of skin. It was pretty bad for a couple of months until I finally started wearing band aids. Then it improved rapidly, but after using some Lotromine and coconut oil, it's not completely healed. Sometimes it almost disappears completely, or it remains a small spot that feels almost like dry skin. But after something irritating happens, it returns in the form of red irritation. No blisters or anything at all. But I was wondering if it could be an STD. I am circumsised and I have had a couple of single one-night stands with two different women. The last one being well over a year ago. No discharge or any symptoms at all before this.

Do you think it's just a mild fungal infection that's difficult to heal because of how fragile the glans skin is or is it possibly an STD?

Brief Answer:
Unlikely STDs.Please send clear photos.

Detailed Answer:
Hi
Thank you for query. I am Dr Bharatesh D Basti answering your query.
Welcome to health care magic. I have gone through the details and photo.

The photo is not clear,it would be better if you can send clear photos for clarification.
It appears to be due to fungal infection with the given photo, and given description.

Wear loose cotton inner wears.
See that there is no friction or irritation.
Clean area with warm water twice daily.

STDs after 1 year exposure with such manifestation is unlikely.
But it is better to go for STDs test as you had multiple partners.

You can get back for any clarification with clear photos.

Sorry for the inadequate lighting/focus on the previous photo. Here's another one. It looks a bit different than earlier. It seems to be doing better since I have started using scented soaps and turning down the water temperature.

In your experience, does this condition seem to be somewhat common in healthy people? Is it likely that injury allowed jock itch to spread to the glans? I'm just a little concerned because I read that this type of thing is uncommon in circumcised males.
Does it typically take months to heal to the point that it doesn't reoccur?

Brief Answer:
Fungal infection Known for recurrence.

Detailed Answer:
Hi
Thank you for follow up query.

Yes. it is less common in uncircumcised men.But a common infection in many individuals. They can spread from other areas(groins).Maintain genital hygiene.

Candidal or fungal balanitis usually resolves in few days. They are known for recurrence. sometimes they remain persistent.

Check for diabetes as it may be the reason for persistence.
You may consult your doctor if persistent for smear examination, or biopsy .

Not to argue, I'm just afraid I'm not effectively communicating. It doesn't seem like this is balanitis so much as it's friction irritation as I can put a band aid on for a couple of days and it disappears. If I masturbate or get an erection for a prolonged period of time, it comes back in the form of three dots. Basically outlining the original wound. The dots generally feel like dry skin. The last photo I sent was shortly after I had an erection. Ill include a new photo tomorrow.

It's doubtful that I'm diabetic as there's no genetic predisposition and im very active and diet conscious. Again, not trying to go against you, I just want to make sure I'm being thorough. Could this just be irritation and cracked skin that would heal if I could manage to leave it alone for a while?

Brief Answer:
Yes may be due to irritation or friction.

Detailed Answer:
Hi
Thank you for follow up query.

Yes we cannot be conclusive with the details given.
It is many times difficult to diagnose when we examine such cases and treat.

Yes if it is not responding then it may be not be fungal infection too.As I have mentioned earlier you may need to get examined by a doctor and get smear test or biopsy done if not responding.

As you have pointed it may be due to irritation or friction or over usage of cleansing agents too.

You can just stop applying all ointments and cleansers.
Clean area with warm water twice daily.
Avoid all friction or irritation or masturbation.
Observe for few days.
If responds well and good.

Otherwise you can consult your doctor for examination and further tests if needed.

You can get back for any clarification.

Last question and then I'll rate you as you've been very helpful. Do you think these characteristics are more indicative of fungal infection or contact dermatitis? My skin has always been notoriously sensitive. Thank you for your time.
