Suggest Treatment For Persistent Heart Burns

Question: I need an advice to get out of my heart burning sensation which I have for years.

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist. I understood your concerns. It is good to hear that endoscopy was normal. Your symptom suggest that you are have Reflux disease which is non-erosive. I will advise you to:

1. Avoid fatty meals
2. Don’t lie down soon after meals for at least two hour
3. Take small and frequent meals, instead of full meals at a time
4. wear loose clothes
5. If you are overweight or obese do weight reduction with dieting and exercise
Among the medications:
1. I would suggest the use of levosulpiride. It don't simply act as a prokinetic agent but it also increase the tone of lower esophageal sphincter
2. Baclofen also helps in increasing tone of lower esophageal sphincter
3 Take ilaprazole which has better efficacy, and I found it very useful in my patients with refractory reflux disease
4. Also add tablet famotidine at bed time
5. You may also add tablet amitryptiline at night which help in reduction of hypersenstivity to heartburns, I found it very useful in my patients with refractory reflux disease
You may discuss with your doctor regarding these medicines and get prescription of these

Dear Doctor,

Thank you so much for your advice. I have following questions below:

1) levosulpiride - Is it a tablet. If yes when should I take it?
2) Baclofen - Is it a tablet. If yes when should I take it?
3) ilaprazole - Is it a tablet. If yes when should I take it?
4) famotidine - Should I need to take before or after dinner at night
5) amitryptiline - Should I need to take before or after dinner at night

All those medicines mentioned above, how many days should I take it?

Ilaprazole and levosulpiride comes in capsule form in combination to be taken half hour before breakfast
Baclofen is a 5mg tablet to be taken thrice in a day
Famotidine 40mg tablet to be taken after dinner
Amitryptiline 10mg tablet to be aken at time of sleep

I would like to know, till how many days I can continue to put these tablets. What is the advise after that?

Also Baclofen tablet should I need to take after or before food.

Welcome back. Sorry for delay. This all medicine you may take for 4 weeks then according to response few of them may be stopped. Baclofen can be taken at any time.
