Suggest Treatment For Persistent High BP Readings

Question: I have had consistently high blood pressure for the las 3 months. It ranges from 160's to 200's over 90's-124. I take atacand32mg and demadex10 mg. should I change medicine? I have feelings in my feet like they are on a vibrator and my toes twitch involuntarily. Sometimes my feet burn so bad I can't sleep. Is this neuropathy? I have diabetes a1c was 5.7. I have dysautonomia. I have been nauseated for the last 2 months. Ct scan, blood t sets, stool samples are all normal. Sometimes I throw up til my nose bleeds.

I read your question carefully and I understand your concern.

Those blood pressure values are indeed very high and since the situation has persisted for three months it is obvious that something must be changed with your treatment. I wouldn't say that the medication you take must be necessarily changed, especially Atacand I would leave as it is, but a new medication must be added. You take two classes of antihypertensive drugs, an ARB (atacand) and a diuretic (demadex). There are many other options, but the two most used groups are either a representative of the beta blocker class (like metoprolol) or of the calcium channel blocker class (like amlodipine).

As for your burning feet, from your description it looks typical neuropathy, if you have long standing diabetes that should be the cause. Antiepileptics are first line treatment for that, but while trileptal is not a bad drug, I wouldn't say it is a first choice drug, recommedations point at Pregabalin (Lyrica) or Gabapentin (Neurontin) as the most effective choices for diabetic neuropathy. Apart from them there is also the antidepressant group some of which may be used for diabetic neuropathy like Duloxetine.
