Suggest Treatment For Persistent High Fever

Question: My five year old son had fever for last 3-4 days and doctor had prescribed the ibusesic plus. We gave it for first 3 days but fever was coming back every 5-6hrs, so we started giving Meftal-P after consulting his regular paediatrician. Later she prescribed the test in which result ESR was 50 and plate count 30.
Today our regular paediatrician gave him Taxim - O and Meftal - p.

Based on her ESR observation she advices to get the tympanogram and mantoux test. She said my son may have cold related complications which should be checked and then if required some kind of nosal operation or medicine will be prescribed.

I am little worried, can you please advice.

I have gone through the details provided by you. Although you say that your son has been suggested to undergo a tympanogram and that a nasal operation has been suggested, you have not indicated the associated complaints. Most of the fevers that children get are caused by viruses. Such infections do not cause any definite changes in the blood report. The blood report that you have attached, does not indicate any bacterial infection. So the possibility of a viral infection is high. Such an infection generally persists for 5-10 days and does not need any treatment other than keeping the temperature in control by cold sponging or use of paracetamol (meftal-p). ESR simply indicates that something is wrong and no specific information about the cause is provided by ESR.

Information on whether there is cough and for how many days, would have been helpful. If I could examine the throat of the child, I would have looked for redness and tonsillar enlargement which could be the possible reason behind your doctor suggesting an ENT intervention. Often children have a tendency of the tonsils getting enlarged, which predisposes them to infections. That could be the case here.

It is difficult to comment on whether the child really needed the antibiotics or whether the doctor just wanted to be on the safer side. But since you have started the antibiotics, continue till the full course is completed. In case the fever does not get relieved by the 7-10th day, I would insist for a Widal test or a Typhidot IgM test to be done, although I do not think that would be required. No need to be anxious at the moment.
