Suggest Treatment For Persistent Lower Back Pain And High Fever

Question: I am diagnosed with a serious kidney infection with a low white blood cell count. the CT scan shows inflamatin of my kidneys with fluid on or in my uterus. I am on antibiotics which seem to be helping a little but I am still hurting in the lower back area with chills and high fever.

First of all, it is sad to know what you had gone through. Based on the history you have shared, it seems as if these all symptoms are due to primary infection that you have already been diagnosed with. the way you have described it, either it is pyelonephritis or hydronephrosis as inflammation or swelling of kidneys is concerned. Now It will take lomg course of injectable antibiotics and will take time to control it completely but good sign is that you are also noticing that they seem to be working. Kindly give them some time, but if fever persists even after 72 hours post current antibiotics combination, kindly review to change the combination. The uterine inflammation or fluid around it needs further evaluation by a gynecologist as to whether it needs surgical intervention or not.

It's advisable to be on injectable antibiotics under critical monitoring, which should be broad spectrum and more guided approach would be to have urine, blood and uterine or cervical swab cultures to identify the exact causative bug and what antibiotics is it sensitive too. Daily complete blood counts to look for white cell count alongwith CRP is better to monitor the improvement in infection or otherwise. If needed CT scan of pelvis may also be needed to look for the extent of infection.
