Question: From last 3-4 months I am facing issue of memory loss. I am getting trouble in recalling things

Hello,
You definitely have vitamin D and vitamin B 12 deficiency.
You need to take regular supplementation for few months to an year.
For vitamin D you can take weekly XXXXXXX or Syp of vitamin D 60000 units or intramuscular injection once every 3 monthly.
For vitamin B12 you may get injection containing 1000 micrograms as per schedule or take oral supplementation containing atleast 1500 mcg daily.
