Increase in nipple size in old age with no other symptom or discomfort or any asymmetry is likely a benign cause most probably physiological gynecomastia. Age related hormonal fluctuation is the commonest cause. Other possibilities are age related fat deposition in glandular tissues of nipple can also cause this nipple size increment.
