2. How is the attaining of developmental milestones
3. Is the stool oily and floats in the pan
4. Is there any color change in the hair or skin
5. Is there any history of recurrent serious infections other than Pneumonia
6. Were the two pneumonias demarcated on chest X-ray
7. Was there clearacne on 1 st X-ray before the 2nd pneumonia occurred.
8. Was it occuring at same place in lungs
Based on the current information the possibilities are:
1. Immunodeficiency either primary or acquired.
2. Congenital lung anomaly.
Two pneumonias occurring 4 months apart definitely needs evaluation immunodeficiency.

His birth weight and current weight are excellent.
It is true that RSV will have a preponderance to secondarily developing wheezing but not to infections, so do not worry. That too after this long period, it is unlikely. Usually secondary infections are more common during the RSV infection.
Croup is a clinical diagnosis and I completely agree with your doctor as nobody can mistake the classical inspiratory noise during the croup. Moreover one has to listen or the see the video of a child to diagnose or agree or disagree with croup. I suggest to keep your son as comfortable as possible and do not make him cry or agitate as these will aggravate the noise and croup.
