Suggest Treatment For Polycystic Ovary
Question: Hi Doctor. I have 1 polycystic ovary and 1 normal ovary and a 35day cycle on average. But I was menses did not come for the month of September. On Day 45 I took pregnancy tests both urine and beta hcg blood, both negative. So I saw my OB. Based on transvaginal ultrasound done last Sept. 29 Cycle Day 45, there is a 1.7cm dominant follicle with chance to mature so OB recommended intercourse every 2 days with follicle scanning. She also prescribed Duphaston for 2x/day for 10 days. I had sex with my fiance every other day from Sept. 29 until Oct. 5 but failed to see my OB for further follicle scanning. I also religiously took the Duphaston because my OB assured me that if I ovulate and conceive, it will actually help support the pregnancy by enhancing endometrium. If no conception occurs, my menses will come about a week or 2 after my last dose of Duphaston. I had very watery and seemingly fertile cervical mucus the whole week so I took that as a good sign. Then on Oct. 4, Cycle Day 50 which is my 5th day of Duphaston, I experienced spotting. I felt bad because it might be mens meaning no baby. Then it became light to medium bleeding for the next 3 days. Obvious flow similar to mens but not enough to soak a sanitary napkin, 1 unsoaked napkin a day only. Is this considered mens already or just breakthrough bleeding? Is it still possible that I conceived? My OB told me to see her after my last dose of Duphaston which will be tomorrow but I am so anxious I can't seem to wait for definitive answers. Please give me your opinion. Thanks.

This spotting is not a good sign and it is due to your periods which has most likely reduced because of duphaston.
Most probably your gynecologist will stop duphaston and within a day or two of stopping it you will get full flow of your periods.

so is it very certain that i am NOT pregnant and that our timed intercourse for the whole week was not successful? no need for pregnancy test next week and useless to hope for a positive one?

what do you think happened to the dominant follicle during my transvaginal ultrasound? if my fiance's sperm and everything related to fertility is normal, what could have happened to the dominant follicle? what could have caused the failed intercourse that we did every other day for the whole of last week? so i could avoid failure next time.

In a normal couple with ideal test results and timed intercourse, chances of pregnancy are only 20-25%.
So even if your dominant follicle had ruptured properly and semen was normal with duphaston chances are not more than 40%.
From next time onwards get a proper follicular study with ovulation induction from a good infertility specialist.
Don't be anxious as fertility treatment at times takes a while to give results but if all tests are normal, results are not that far away.

thank you doctor. we have been trying to conceive for 9 months now, this october is the 10th month and it is very disheartening to see negative results all the time with pregnancy tests. but we have not sought help and thorough fertility work up with an infertility specialist yet because we were told to give it a year because i am under 35 years old (30 to be exact) and my fiance is 28. and i am also on the heavy side so my ob told me to lose weight as just as little as 10% weight loss would help boost fertility. is that true? she also gave me the option to try clomid if i am in a hurry to conceive but my fiance is wary of synthetic treatments without me trying to do everything natural first like good diet and weight loss. he is also going to do healthy lifestyle changes to motivate me and show support. and by the way is clomid a factor in breast ca development? i certainly have the genes for it because my older sister is stage 2 her2 positive but in remission now for 7 years already. she is hesitant to recommend clomid for me because she attributes her breast ca to her clomid intake many years ago to help her conceive her daughter. what is your opinion on clomid and breast ca?
