Suggest Treatment For Premature Ejaculation And Back Pain After Intercourse

Even though I can control ejaculation during foreplay but tend to ejaculate within 1 to 2 min after entering and with few strokes or even without stroke (3 min max) I have tried all the techniques of Start stop, squeeze, Kegal, Local numbing creams, hypnotism therapy etc. for more than 1 year and not cured of from problem.
I have observed my Penis head to shaft joining area is very much sensitive and ejaculate with very few stroke. (I have No problem of frenulum stretching)
Also If I have sex for continuously for 3 to 4 days ( or even alternate days) my lower back starts severely continuously paining and continue for 15 days. and need 2 to 3 months to cure completely.
Premature ejaculation I observed almost 10 years ago, At that time not worried about it as I read somewhere that PE will get cured automatically after some years 30 of age, but started worrying about it after repeated failure. (Since my partner is supportive and satisfied with foreplay t, but this bothers me.
For back pain: I have no problem of Slip disc or nerve stretching
Back pain severity will be reduced slowly with IFT stimulation TENS for a one to two week
About my food and exercise habit (when in free of back pain): Daily joggs for 2 km, Light exercise,No alcohol,No smoking, I Eat well balanced food, drinks min 1 to 2 litre water everyday, sleeps for 7 hours, BP and sugar are normal,

I read your query and understand your concerns. As you have used all behavioral methods there is option of moving to medications.
The medication useful in Premature ejaculation include as follows.
SSRI antidepressant drugs cause delayed ejaculation as a side effect, and this side effect is used therapeutically in the treatment of premature ejaculation. These drugs are as following:
Tablet. Paroxetine [paxil 25 mg] at night or
Tablet. Fluoxetine [Prozac 20 mg] in the morning after the breakfast.. or
Tablet. Sertraline [Zoloft 50 mg] at night. Increase the dose to 100 mg after 15 days.
A person with premature ejaculation need to take any of these drugs regularly and its effect will appear only after 15 to 20 days.
Another strategy is to take drug one hour before sex. The another antidepressant drug dapoxetine [sustinex 30 mg] is given one hour before sex and it cause delayed ejaculation and prevents premature ejaculation.
Regarding back pain although this is not my expertise but I feel the faulty technique of having sex could be responsible for this as only sexual intercourse brings this pain and no other sternous activity. So having a different position while having sex is most likely to give relief from back pain.

Regarding PE I have tried earlier the tablet given one hour before sex (tried only 2 occasions) that also didn't worked for me. I forgot to mention this in my earlier mail.
1) Regarding tablets Paroxetine or Fluoxetine or Sertraline, is it requred to take continuously for months/years ?
2) Is there are short term or long term major adverse health effects for Liver, kidneys, other glands. etc.. and how sever if it is??
3) Since these are antidepressants will it cause addictions in long run?? (like narcotic drugs)
4) Are all these drugs available in India? is dr. prescription letter required for same? request to provide letter.
5) Lower back pain is not due to wrong position of sex, as even a simple masturbation (1 time a day) for continuously or on alternative days for 4 days cause me severe lower back pain. could you suggest the specialist dr. to deal with this type of problem (neurologist?/andrologist?/Ortho?..etc.)

If the one hour before sex medicine did not work earlier the dose can be increased to 60 mg as it is recommended dose. Generally 30 mg is starting dose and depending on the response the dose can be increased. Another thing it should be taken at least 90 minutes before sex and preferably 2 hours.
Yes all medicine mentioned in the query are available in XXXXXXX and mostly free of any major side effects. There are certain chronic side effects but they are rare except erectile dysfunction which can occur in above 10 % of recipients. There is no side effect on liver kidney and glands.
They can be taken for six months and behavioral techniques need to be continued to have long lasting effects. After the six months a gradual reduction can be attempted and depending on the response further decision should be taken.
No they do not cause any dependence. They can be stopped at any moment as per the guidelines applicable differently for different medications.
Prescription is required and that can not be delivered online without physical examination of patient. For same you need to visit a doctor in your city.

Along with tablet is it ok to use cream numbing cream for penis? and can you suggest a better cream/ for the same

Although it is ok to use cream but it is neither necessary nor advisable. The use of cream, which is anaesthetic preparation based, kills the purpose of sex and I never recommend anything for a person who has at least one to two minutes.
The cream is advisable for a person who get discharge within seconds or even before penetration.
