Suggest Treatment For Premature Ejaculation When On Hypertension Medications

Question: Hello Doctor,
I am 32 years old and married for 4 years and have a 3 yrs old daughter. I get ejaculation within 5 to 10 movements while having sex with my wife. I do not have any problem to get a child birth but I could not have sex for a long duration. I get erection very normally and stay for a long time before sex and during foreplay but the moment I insert and start moving I get ejaculation within 5 to 10 movements.
At the age of 28 I was asked have medicine for hypertension so I am having "asomex 5 mg" at 9 pm every day.
Please let me know whether my sexual life is normal it is a premature ejaculation ? What kind of treatment I can have?

Premature Ejaculation-PME is defined as ejaculation of man occurs before female partner reaches to orgasm during intercourse.
There is no specific criteria or timing what is called normal duration for ejaculation.So it is subjective diagnosis.If your wife says that you are coming early before she reaches to orgasm ,you are suffering from PME. However if she doesn't complaint but you feel that it is so early than the previous duration then also you can go for treatment.

If you are suffering from, there are two types of treatment.

1. Psycho sexual counselling cum psychotherapy for sex:
Sex is act of two.So preferably your partner should be involved in counseling, that's why it is called couple therapy.Stop and squeeze technique and Master & XXXXXXX technique are specifically useful which can be learnt from psychiatrist/qualified sexologist.

If you have any use of smoking,tobacco or excessive alcohol, please stop that.

2.Medicines: Clomipramine (Dose 10-25 mg), Paroxetine (10-20 mg) and depoxetine(30 mg) are useful.
Clomipramine or paroxetine can be taken initially daily and later on just before 2-3 hour of sex under doctor's guidance.While depoxetine should be taken 2 hours prior to intercourse only, however availability of depoxetine is the issue.
If I were your treating doctor, I would go with 10 mg clomipramine to start with.
No significant side effects of these drugs.

Another option is use condom so sensitivity decreases and you can last longer.

Considering your age I would suggest go for counselling and behaviour therapy first.

Asomex(amlodipine) can cause issue with erection but in your case it is not so as you are getting erection.
