Question: Hi I am very concerned about my right kidney. I have passed two and am passing two more kidney stones as we speak. In the report it states I have more in the kidney the largest of which is 8mm plus a 6.35 x 5.57 x 8.27 cm calculus in the upper - mid pole of the kidney. I am scared this issue has been going on too long

Hi
From your account, it looks like you are a metabolically active stone former meaning your kidneys have a tendency to form stones despite what would be a normal diet and fluid intake for another person. The good news is that your stones are not to big if the largest is less than 8mm and if you have passed a couple you should be able yo pass these as well.
You can be started on drugs like Flomax which help relax the lower ureter and increase the chances of stone passage. Drugs like buscopan will help reduce colicky pain.
To reduce chances of stone recurrence, you can attempt to improve your fluid intake to over 4liters per day. Restrict salt in your daily diet to less than 5grams per day. Take a diet with low red meat, low oxalates meaning reduce spinach tomatoes rhubarb nuts.
If stones still do form despite all these precautions, you might layer need a detailed evaluation why you form stones. This could include a 24 hour urine collection for calcium,oxalate, citrate and other salts and blood tests for calcium, uric acid, sodium and other substances which might guide us as to whether you need oral potassium citrate or hydrochlor-thiazide which inhibit stone formation.

Yes thankyou Dr very helpful. However I do have multiple stones and have been advised of an 8mm stone. Also the question of culculus in mid upper pole? Is that a measurement of the kidney itself or another stone?

I guess you should be concerned about your 8mm stone. Under normal circumstances the chances of this passing out on its own would be less than 50% but in patients who have passed the stones recently the passage would have been dilated enough yo let this pass without hindrance. Flomax is an alpha blocker which opens up the passage further. But obviously you need to consult your local urology team for a prescription for the same and any further advice.

Thankyou again. As noted in my profile I am taking flomax. Still no answer about the calculus in the upper - mid right pole measuring 6.35 X 5.57 X 8.27 CENTIMETERS not mm please advise: my ct scan reports are available in my profile: this was my main question and concern

Hi, The response was after perusing your reports.
As you might have noted, the second scan says 8 mm stone. There is practically no chance of a 8 cm stone any way in your case. So most likely it could have been a printing error or an error by the transcriptionist which is the bugbear of radiologists worldwide.
I also did note the location, i.e. upper mid pole, and that is why I am relatively optimistic of chances of stone coming through in your case, especially as the second scan also suggested mild dilatation of the system which means the path is wider for the stone to pass. A stone in lower pole has lesser chances of coming out spontaneously. This is basically due to gravity and drainage issues of the kidney and you look like your stone is in a favorable location.
