Suggest Treatment For Protruding Hemorrhoids And Blood In Stool

default
Posted on Mon, 5 Jan 2015
Question: Hi, I have protruding hemorrhoids. They started about 8 months ago. I noticed blood in my stool one day. I saw a doctor then and obtained symptomatic treatment. The bleeding lasted 2 days. Since then, although they protrude, I experience no pain or itchiness. Yesterday & today again I had blood when passing stool and a veinous mass came out when I was washing. This is the only occurrence after the initial episode. Today I started on a stool softner, the bleeding will stop in a day or two. The protrusions are pinkish and shiny. Currently I have no pain or other discomfort. My questions: Can I leave this unattended? Do I need any medicines (last time I was prescribed antibiotics)? What are my treatment options, besides surgery?

Brief Answer:
Laxative, Sitz bath, avoid long hours of standing and straining.

Detailed Answer:
Hi.
Thanks for your query, an elucidate history and a clear picture.

I can understand your concern about avoiding the surgery even at this stage and your wish to undergo treatment other than the surgery.

To recapitulate: started 8 months ago - bleeding for 2 days - symptomatic treatment -manual insertion - symptomatic treatment with laxative and antibiotic - Since then 'although they protrude' no symptoms of pain or itchiness or bleeding - now 2 days bleeding & venous mass came out - pinkish and shiny ( seen in picture) - started laxative and hoping bleeding will stop in a day or two ...

Your Query is related to :
- Can it be left unattended
-need of medicines
-Treatment options beside surgery

With the picture you have provided, these look to be in need of treatment. They are prolapsed. If the y remain outside or come outside even after being manually reduced they can not be left alone. This is because there are chances of complications of thrombosis and infection.

Medicines: you have already started laxatives, please continue in a dosage that makes the stool soft and not loose. The dosage is person-dependent, meaning you have to decide your dosage after permutation and combination.

When there is a bleeding, there is a breach in surface and stool has the maximum number of bacteria, hence it is wise to take antibiotic and metronidazole. In addition you can take probiotic.

The treatment options other than surgery at this stage are :
Continue medical management.
Diet as suited.
Sitz bath ( in a tub with warm water at least twice a day)
Avoid continuous standing, strenuous work.
Relaxation in a lying down position.
You may discuss about some medicines like Oxerute if it is available in your country.

You may consult a Proctologist and discuss about banding, if he is willing.

I hope this answer helps you, please feel free to ask for more or to clear any communication gap if you feel so.
