Question: Hi Doctor i am attaching the picture of my sisters feet ..can u tell what exactly is this ...can this be treated via home remedies or we need to go to some specialist ...is there any specific medicine for this.

Thanks for being at healthcaremagic
I have reviewed the attached photographs and there are few possibilities: either hyperkeratotic eczema or psoriasis.
This can't be corrected with home remedies.
I need to ask you few questions:
What is the extent of itching?
Are both the feet affected?
What is the duration of the lesions?
Is similar lesions present elsewhere on the body?
Kindly revert with the answers.

Thanks for your answer ... answer for ur question are
What is the extent of itching? too much
Are both the feet affected? yes
What is the duration of the lesions? mostly in winter time
Is similar lesions present elsewhere on the body? yes on palm of the both hands just started ...

additionally on feets there are wounds and also some fungus type things...

Management of hyperkeratotic eczema/ psoriasis

Detailed Answer:
According to your complaints it seems to be either hyperkeratotic eczema or psoriasis.
If I was your treating doctor I would recommend you to apply topical antibiotic like mupirocin cream on the cuts twice daily followed by application of a steroid ointment like clobetasol propionate ointment on the affected areas twice daily.
After 15-20 minutes of application of steroid cream apply a moisturiser containing urea and glycolic acid like xerina cream or moisturex cream on the entire hands and feet.
For itching take tablet fexofenadine 180 mg twice daily.
These are chronic diseases and treatment needs to be continued for a long time but under the supervision of the doctor.
These are prescription drugs and she needs to be the doctor in person to get them.
