Question: I don't know if anyone can help me as I seem to have baffled health care professionals so far. I have a pmh of non specific colitis of the terminal illium, pylonephritis, reaccurent uti, pleurisy, reaccurent tonsillitis, night sweats, vit d deficiency, folate deficiency, b12 deficiency and fatigue. I have a family history of rheumatoid arthritis and psoriatic arthritis. Tonight my left hand was red and inflammed at the joints followed by it swelling up and it hurts. Does anyone have an idea what's going off with me I know there's something not right with all if this has happened over the last 4 years

Brief Answer:
There is a possibility of Psoriatic arthritis.

Detailed Answer:
Dear Sir,

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that people who have a family history of Rheumatoid arthritis or Psoriatic arthritis are quite prone to get these disorders . The colitis of illeum , UTI can be linked and the swelling you are getting can be a part of Seronegative spondyloarthropathy spectrum.

So most likely it has to be psoriatic only but since RA is also there in your family , It is advisable to rule that out also . Get a RF, Anti CCP antibody , HLA B 27 testing done . I would like to see your previous test reports if you have any . Also mention your age and sex . I think you should see a rheumatologist as physical examination will be important in establishing the diagnosis .For swelling in wrist you should take naproxen 500 mg twice daily after discussing with your treating doctor.

Take care.

Detailed Answer:
Dear Sir,

When you have a person suffering from autoimmune disease in the family the probability of having similar disease goes up . You have sent me complement levels which are within limits so nothing much I can say . For lupus evaluation ANA testing remains the most important test . If your rheumatologist has done that please send that to me for review.
