Suggest Treatment For Psychosis With Depression

Question: Hi I suffer from psychosis with negative symptoms and also major depression. I'm currently on 400 mg of quetiapine at night and 25 mg of quetiapine and 40 mg of fluoxtine in the morning. I seen my GP last Friday and he pressured me to go back on 40 mg of fluoxtine in the morning that i have have already taken for some time along with 400 mg of quetiapine at night and my condition worsend and I wasn't feeling good at all. I was on 30 mg of fluoxtine in the morning which I was going a fair bit better on, but was still struggling with depression a fair bit. I've been taking 40 mg of fluoxtine since again since Saturday morning and have already started to feel like I did previously on these medications. I suspect I'm probably better off trying 35 mg of fluoxtine in the morning instead. I can't get in to the doctor's surgery where my GP is until Monday next week. I could go to another doctor's surgery, but I'm concerned they won't do anything for me.

Hello thanks for asking from HCM

I can understand your concern. You have psychosis with depression and are on 400 mg Quetiapine in night and 25 in morning with Fluoxetine 40 mg. On increasing the dose of Fluoxetine from 30 to 40 since Friday you're not feeling good.

Fluoxetine is a SSRI drug and it is expected with this drug to see worsening of symptoms when the drug is first started in an individual or doses are increased. So if you are not feeling good then you can reduce the dose to 30 again. This will be a better thing till you see your psychiatrist.

Even on 30 mg Fluoxetine your depressive symptoms are persistent. In such case I'd advise you to talk to your doctor and discuss about changing the drug. Other SSRI drugs like Escitalopram or Paroxetine can be tried. Escitalopram will be better tolerated and high doses can be given to treat depression symptoms.

You can use supplements like Omega 3 Fatty Acids and Vitamin B12 along with medicines. The supplements will augment the actions of antidepressant drug and you should feel better. Please discuss about these supplements too.
