Detailed Answer:
Physiologic pubertal gynecomastia usually resolves and merits no further evaluation. Observation is the best approach during this time.
But if it persists into adulthood then other causes must be ruled out such as hyperprolactinemia and dysthyroidism, liver and kidney problems, and testicular tumors. About 25% of the time no cause is found.
Surgery is not recommended during adolescence until adult testicular size is attained because the gynecomastia can reoccur if it is done before puberty is completed.
