hello, the rash is ONLY on my inner thighs near my underwear line. i am overweight, obese in medical terms and it is very hard for me to reach to properly dry my self after urination every time. if i am at home it is usually ok but occasionally I am somewhere else and must urinate. could the rash be hygienic due to that? if I use Triamcinolone oinment it clears up about 90% is that what I should be using? please tell me what you think it might be and what meds or ointments I should be using. please let me know if you have anymore questions. thank you, I appreciate your help very much.

Looking at photograph and your description this could be irritant contact dermatitis which can be caused by Ammonia liberated from urine.
Another possibility is eczematous dermatitis or psoriasis

Here I would like to know following details
Since how long you have this problem
do you feel itching in the area?

I would advise you to clean it properly every time you use wash room
You can apply antibacterial ointment or steroid preparation when it flares up
You can continue with triamcinolone ointment.
Let it heal first with a course of antibiotics under supervision of your doctor and keep it clean and moist
stay indoor until it heals completely
