Suggest Treatment For Rash On Buttocks And Groin Area

Question: Hi Doctor - I get repeated fungal infections in groin, buttocks area. It started around my adolescant age (15 yrs back). I went to a doctor at that time and he gave me some ointments. It was all OK and then I got it back in a month or so. I started treating it with Itch Guard or Sapat Lotion (what ever is available at the medical shop). It gets cleared and re-appears in couple of months. Earlier, it used to be in the groin region predominantly, now the focus is shifted to buttocks region. I no longer get infection in groin region.

I lived in US and UK for few years. I dont get this infection that often. Once in a year in US and the infection never happened in UK. Back in XXXXXXX its a frequent occurance. I want to get rid of this fungal infection completely. Please suggest a treatment.

I use to workout heavily until few years back and in recent years into running. So, I have a pretty regular workout regime and I sweat a lot. I wear running tights to avoid rashes in inner thighs.

I have taken note of your query and I have understood it.

Recurrence is common in cases of fungal infections.
The factors that favor a recurrence are: inadequate treatment, inadequate duration of treatment and favorable/ conducive conditions for the fungus to grow.

Treatment must be with both topical as well as Oral antifungals and it should last at least 6 weeks for an adequate duration.

Favorable conditions that favor regrowth are: like being overweight and thus having deeper body folds which traps sweat and warmth and thus provides an Ideal environment for the fungus to regrow, specially in warm and humid weather in summers.
Diabetic patients too have frequent recurrences. You may take up a blood sugar test.

I suggest you to take an Oral antifungal e.g either tab. fluconazole or tab. terbinafine Or tab. itraconazole for at least 6 weeks.

Besides you must also use a topical antifungal e.g either terbinafine or sertaconazole or bifonazole etc, twice daily for 6 weeks minimum. May extend it for another 2 weeks.

Once the condition improves using an antifungal dusting powder in body folds would keep folds dry and thus help in preventing a relapse of infection, specially in tropical weather conditions like in XXXXXXX

General measures like bathing twice daily, wearing loose fitting cotton undergarments etc also help.

Hi Doctor- Thanks for the detailed explanation. I will do as you said. Please give me the dosage of medicines. When should I take it and if it is before or after food. My sugar levels are normal.

Hi.

Fluconazole is usually given as a 'once weekly dose' whereas terbinafine and itraconazole is 'once daily dose'.
Dosages of each of these oral antifungals are standard in all patients..
The duration of treatment with oral antifungals is 4-6 weeks.
Oral antifungals are prescription medications and therefore ideally you need to talk to a doctor in your vicinity for a written prescription.

Topical sertaconazole Or bifonazole Or terbinafine are also prescription topical antifungal medications. They are not available over the counter.
They need to be applied twice daily for 4-6 weeks.

Clotrimazole antifungal dusting powder can be used after 4-6 weeks of Oral and topical antifungals.
It would help prevent a relapse by keeping the folds dry and also being an antifungal would prevent the fungus to grow. It is available Over the counter.
