Suggest Treatment For Rashes On Buttocks And Inner Thighs

Question: The problem I am facing is that there's is a rash between my butt cheeks and it always feels moist and like clear liquid dripping down. Also there is a rash on my inner thighs and under my private area. I did have pilonidal cyst surgery and year ago with open wound healing. Ever since then I have had this rash between my butt. Just last month I had a flap procedure don't for the pilondial cyst because there was still tracks in there. The rash seems to get worse has I move around like being at work and running around. I did go to my primary care doctor who told me this is a yeast infection two days ago. She gave me some cream (anti-fungal). I just want a second opinion on this. I consistently feel itching and like a dripping felling down my crack. Also the rash runs from the top to the bottom of my crack.

Hello. Thank you for writing to us.

I have gone through yur query in detail and i have understood your concern.

I believe the condition is primarily Intertrigo, which is inflammation of body folds. Itching is due to superimposed yeast infection. Moisture from sweating and secretions from sinus also favoring yeast infection.

If i was the treating doctor i would also suggest you to use a topical antifungal cream, twice daily.

In addition I would also want to start you on oral antifungal e.g weekly fluconazole.

Topical antifungal dusting powder, in between cream applications would keep the folds dry and prevent fungal/ yeast proliferation.

An otc oral antihistamine e.g cetrizine 10 mg once daily will provide symptomatic relief from itching.

Thank you for your response. I'm just concerned about the feeling of liquid between my cheeks . it almost feels like its sweat. sometime I do see a clear droplet fall down my leg. Its like looks like water. Can a yest infection cause excessive sweating? And you don't think this has anything to do with the trauma from the pilonidal cyst surgery?

First sinus should heal and stop discharging completely

Detailed Answer:
Hi.

If the pilonidal sinus is still active and discharging it is most likely the reason for the feeling of moisture between folds, dripping and also resultant yeast infection.

Yeast infection by itself will not cause dripping.

The foremost thing would be to manage draining sinus with antibiotics and allow it to heal completely so as to prevent secondary problems like yeast infections.

Unless the sinus heals and stop discharging completely, topical and oral antifungals and antifungal dusting powder will provide only temporary relief from yeast infection.
