Based on the history you have shared, and the image that you have attached, it seems like infection of skin folds called intertrigo likely due to fungal causes or superinfection by a bacterial infection as well. It's better to keep the skin dry even with corn starch or anti perspirants and using topical antifungal like clotrimazole and antibiotics like fucidic acid along it.
For itching oral anti histamines like cetrizine can be used. Avoid using under garments for few days and get it reviewed atleast once by a dermatologist for need of cultures from the site of lesion if prescribed.
