Suggest Treatment For Reactive Arthritis And Intermittent Black Stool

Question: Good morning doctor,
I am 28 years old, I am suffering with reactive arthritis, I am using methylprednosline 8mg once a day,
sulfasalazine 500mg 3 times a day and etodolac(Lodine)+Paracetamol twice a day. But doctor, I am using this medication since 15 day, the moment i started using my stool was sometimes black, tar color and sometimes normal yellow, No bad smell, nor severe pain is there. 6 out of 10 times its black. I used iron tablets and stopped a month ago. I am bit worried, my weight is same, appetite is good, no weakness nothing, No stickiness of stool and it coming out normally as well. So please tell me why the change in color was. Thanks in advance

Dear Sir,

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that it does not seem to be related to side effects caused by these drugs . I will suggest you to get stool for occult blood and stool routine microscopy done to see if blood is not causing this. Also since you stopped taking iron tablets 1 month back , it doesnt seem to be related to that also.

You can discuss this with your treating doctor also.

What are chances that it is related to ulcer?
What are chances that it is related to cancer?

If its blood it has to be alwyas black na? but for in a day I have one black stool and second will be normal stool? No pain also? I used nitrofurontoin and using it since 2 months, used pregabalin and methylcobalamin for 2 months an stopped a month back, Took becosules tablet for 5 days from XXXXXXX 9th to XXXXXXX 13th and now taking once in two days.

Possibility of ulcers can be there , further colonoscopy should be done.

Dear Sir,

Thank you for reverting back.

There is always a possibility of intermittent bleed so its not always that blood has to be there in the stools . The possibility of cancer is quite low as there are no associated signs of weight loss etc. but ulcer can be there although you dont complain of abdominal pain .

Its better to get the tests done as advised earlier , after that you can go for colonoscopy if your doctor advises . Even reactive arthritis is also associated with inflammatory bowel diseases so further evaluation is necessary. Also HLA B 27 gene testing is advisable .
