Recurrent herpes genitalis
Detailed Answer:
Hello,
Welcome to healthcare magic.
I understand from your query that you are having repeated episodes of painful blisters on your penis.
I have seen the clinical pictures that you have attached.
From your history and photographs, I would consider the possibility of genital herpes.
Herpes is a viral infection which is transmitted sexually. The virus stays in the nerve endings and causes breakouts when the body ' s immunity is lowered.
You need to take a course of antiviral tablet called Acyclovir during the active episodes.
However, if you are getting more than 6 episodes per year, you may benefit from suppressive therapy. In this, you need to take Acyclovir daily for 6 months whether or not you have the blisters. This will suppress the virus and reduce the frequency of outbreaks.
Acyclovir is not an over the counter medicine. Please get it prescribed by your dermatologist.
Hope this helps you.
Take care.

Dr. Johny Chacko

Acyclovir dosage
Detailed Answer:
Hello,
The dosage for an active episode is 400 mg 3 times a day after meals for 5 days.
Suppressive dosage will be determined by the doctor based on your clinical response.
There are no major side effects of this tablet. But since it is excreted by the kidneys, you need to take lots of water while on this tablet.
Do consult your dermatologist before starting the tablet. This is not an over the counter medicine and needs a prescription.
Hope this helps you.
Take care,

Acyclovir details
Detailed Answer:
Hello,
Let me clarify your questions-
1. As I have mentioned in my first mail, there are 2 types of acyclovir regimes. One is to treat the active episode what you are currently having. In this regime, on taking the medicines, the active lesions will subside. However, they may recur again as and when the body's immunity goes low. Then, you will have to take the medicine again.
The other regime is suppressive therapy. In this, you continue taking acyclovir daily whether or not you get blisters. This will suppress recurrent episodes. Usually, the course if 6 months. Once you stop, you may still get blisters but the frequency of episodes will come down. This regime is usually given if you are getting more than 6 episodes per year.
2.Yes, Acyclovir could affect kidneys when taken on a daily basis as in suppressive regime. Hence, your doctor will monitor kidney tests once in 6 months.
3. There is not much of side effect on the liver.
4. Upto 2 litres of water per day is sufficient.
5. As I have mentioned previously, the virus will lie dormant in the nerve endings and can cause break outs every now and then. This is something like a common cold which you get whenever body's resistance is low. Hence, taking acyclovir will not remove the disease forever. It will only reduce the frequency of recurrences.
Do consult a dermatologist before starting therapy.
Hope this answers your queries.
Take care.
