Brief Answer:
Restart isotretinoin along with topical medication

Detailed Answer:
Hello. Thank you for writing to us

I am dr.kakkar(dermatologist). I have gone through your query and i have noted your concern.
For back and trunk acne you may use brevoxyl 4% creamy wash for body wash and use cetaphil cleansing lotion for face wash, twice daily.
Topically you may use clindac-A gel for local application during day time over acne affected area on face and back. At night you may use Deriva-Cms gel once daily.
Along with these topical medicines you may restart with isotretinoin 20mg once daily for 4-6months. A lot of acne patients require repeat courses of isotretinoin.

Brief Answer:
I suggest Isotretinoin at a low dose

Detailed Answer:
Hi.

You can take isotretinoin in a lower dose i.e 10 mg once daily. Take it after dinner. Stomach pain is usually not a side effect of isotretinoin unless it has caused hepatitis or pancreatitis.
Acne can persist for years or even decades and it is not uncommon to have acne at your age and many patients may require repeat courses of isotretinoin, specially in those who have moderately severe to severe grade of acne.
