Suggest Treatment For Recurrent Redness And Softness Of Penis Skin

Question: Hello.

I have been having a problem with my penis skin for the last 4 months. My penis skin has become very soft, reddish/whitish and sometimes the white color skin is going off when rubbed with finger. since the skin has become so soft, small cracks can bee seem in times. There is also a severe pain in penis when this condition gets worse.

I had a similar condition about 10 years back and at that time doctor has given me Candid Cream (Clotrimazole 1%) and after using that for 2-3 months I was ok and never had that problems until now.

When this problem started recently I started using Candid Cream (Clotrimazole 1%) again (without consulting a doctor) and used it for 1 month, it reduced the problem by say 70% but I was never fully cured and after few days of stopping it condition was back as it was before.

Then I consulted a urologist, he examined the penis and given me DACTORCORT CREAM and I used it for 2 months, this cream worked slightly better than Candid Cream but it never fully cured me after using for 2 months. As soon as I stopped it the condition was back.

Then I met the urologist and then he gave Aristocort cream 1% and I used it for 1 day and condition got worse so I stopped using it.

I also have no diabetes (checked 3 weeks ago) and circumcised penis. I'm 33 years of age.

If it helps diagnosis I can send a image of my penis with this condition.

It has become recurrent due to inadequate treatment.
A combination of oral and topical antifungal should be used.
I treat such cases with tablet Fluconazole and ointment clotrimazole.
This gives good response within a month.

Thanks for your answer I have few followup questions.

1. May I know the facts that made you diagnosed the problem as a fungal infection?

2. I have included 2 images, does those further confirm this problem as a fungal infection?

3. What type of tests can done or available to confirm this as a fungal infection, does that be needed at this time?

4. You have mentioned that "It has become recurrent due to inadequate treatment.", does that actually mean the creams that I have given (see my original post) is wrong or they are were just inadequate? can it be that the fungal infection got resistant to the treatment used?

5. You have mentioned "You can try this in consultation with your doctor.", what type of a specialist should I consult?

Thanks for the follow up and photograph.
I can see there is lot of scaling on the glans penis which is suggestive of fungal infection.
Regarding your doubts.
1. I diagnosed it as fungal because of typical presentation where only glans penis is involved which subsides for few days with antifungal cream and then reoccurs.
2. I said inadequate treatment because you have never taken oral antifungals. Creams are usually effective but a course of oral antifungal is required for complete cure.
3. I mentioned in consultation of your doctor because you need a prescription from any doctor to get these medicines.

KOH mount can be done at this stage to see fungal hyphae.
Although this test is not must as clinical diagnosis is enough to start treatment.
