Question: Hi I am XXXX age : 42. I am prone to dust allergy and have been treated for allergic Rhinitis in the past on many occasions. I have been suffering from nasal congestion and productive cough for the past 1 week and have been treated on Levoflox 500 mg and Ebast 10 mg in addition to taking Ascoril Expectorant. There was improvement after 5 days, since yesterday the productive cough has returned along with nasal congestion and the phlegm is pale green in colour. I have been given Levoflox 500 mg, Ascoril expectorant and Montelukast Sodium 10 mg and Acebrophylline 200 mg Sustained release tablets.
I am travelling in dusty road conditions with a chill weather prevailing in the evenings.

Brief Answer:
Wear face mask

Detailed Answer:
Hello

Thanks for the query

I understand that you are having recurrent upper respiratory infections, it is probably due to the dust.

I strongly recommend that you wear a N80 face mask while travelling, this will not only prevent dust from being inhaled but will also prevent micro organisms from entering into your respiratory system.

I also recommend you to get a CT of the sinuses to rule out sinusitis, as this also leads to recurrent infections of the upper respiratory tract

I hope I was of help, if you have any further queries please get back to me

Hi Doctor XXXXXXX

Thanks for your response.

While I will wear a face mask as suggested while travelling. The allergy doesn't occur frequently under normal weather conditions.

My present condition is that I am not getting adequate sleep due to the congestion and cough and feel fatigued. Can I go ahead with Levoflox 500 mg, Ascoril Expectorant and Montelukast Sodium with Acebrophylline sustained release tablets to ease the present condition. I need to be on calls often with clients and internal teams at office and the cough is an irritant.

Is there any other way to boost my immunity?

Best Regards.

Brief Answer:
Can use montelukast.

Detailed Answer:
Hello

Thanks for getting back

I am sorry for the short delay in my response, it was a very busy day at the hospital.
Here are my suggestions to you

1. The best way to get rid of congestion is to put Oxymetazoline drops, 2 drops to each nostril 5 minutes before going to sleep. It will decongest your respiratory immediately and the effect will last for 4-6 hours
2. I recommend that you take Tab Azithromycin instead as you have recently used Levoflox, we do not repeat an antibiotic within 6 months of treatment to prevent resistance. It has to be taken once a day before food for 3 days.
3. The reason why you are having cough is because of Ascoril, use ascoril only in the morning and afternoon. Avoid using it at night. It is an expectorant, experortants induce cough to throw out sputum. I suggest lozenges like strepsils three to four times a day instead
4. You can go ahead and use Montelukast with acebrophylline.
5. There is no way to boost immunity, follow a healthy diet. Take Vitamin B complex if you are feeling excessively tired

I hope I was of help and clear in my suggestions

Regards
