Suggest Treatment For Recurrent Urinary Infection

Question: I am a male- 10-12-1948 in good health but have recurrent urinary tract infections for a long time. I am afraid of the bacteria getting stronger and the antibiotics weaker. Is this a good choice. I was hoping for a more permanent solution with a series of medicines that make the urine acidic and turns it into formaldehyde. I heard this works great. What are your thoughts? I have been cleared of any sign of cancer. I have an enlarged prostate; the bladder does not empty properly when I have an infection; I have to use a catheter.

Thanks for your query ,based on the facts that you have posted it appears that you have recurrent episodes of UTI refractory to antibiotics and you are known to have Enlarged prostate with significant post void urine .(PVR)

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) What is the size of your prostate reported on recent Ultrasound Scanning and what is the volume of PVR?
2) What drugs are you taking for enlarged prostate ?
3) Are you diabetic?

The problem of UTI that you have is due to significant post void urine remaining in bladder due to bladder outlet obstruction because of enlarged prostate . This PVR is the source of infection .Hence no matter what antibiotics or other treatment you take infection is bound to bounce back frequently unless the primary cause (Enlarged Prostate) is removed .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

I would suggest you to get your prostate removed by an endoscopic surgery termed as Trans Urethral Resection of Prostate (TURP).

This is gold standard surgery practiced all over the world for enlarged prostate and offers permanent cure and improves quality of life at this age .More over you will be free from getting UTI for forever.

Wow, that is an option that I never occurred to me. That me be the right thing to do. How dangerous is it? Is this major or minor surgery? Those drugs that I am taking for enlarge prostrate are: finasteride, tamsulosin, and another that begins with the letter c.
I am not diabetic. Obviously, I do not want more kids. I have a penile prosthesis which works great. No need to worry about being impotent.

There is another class of drugs which turn the urine acidic and into formaldehyde that have been very successful- no need to take antibiotics because there is no infection. That was the other option. These drugs have different names: metenamina, methenamine, Urex, among others. Have you investigated that succuss of these drugs?
However, your idea may be better. I am currently in Mexico. But, I live in the St XXXXXXX area. But, I will be returning home soon.
Where would be a good place in the St. XXXXXXX area or anywhere else to have that surgery done?

Rectal exam is impossible as I have a colostomy due to colon cancer in 1982. But the other factors in your answer have already been confirmed- enlarge prostrate and inability to evacuate the bladder adequately and completely,
