Question: hii, about 20 years before i had taken surgery of right side parotid tumor. between 2007 to 2008 is devolved again in same place & i had also taken second time surgery of perotid tumor (Non malignant) in 2008. but from 2013 the same tumor is again growing.thus i had been faced surgery twice. pls advise me what should i do now. now my age is 36 years.

Detailed Answer:
Hi,

Thank you for your query.

1. Such a history is suggestive of a benign parotid salivary gland tumor known as pleomorphic adenoma. It would be better to check the previous histopathology reports from the previous surgeries.

2. This is the most common type of benign salivary gland tumour. It has a bosellated shape and hence small area may be left behind if enucleated during surgery without taking out adequate normal tissue around it. This is also common with patient having multiple surgeries over many years.

3. There is a 10% chance of malignant transformation (known as carcinoma ex-pleomorphic adenoma) in about 15 years.

4. Hence get an FNAC (Fine Needle Aspiration Cytology) and a CT + MRI Scan done.

5. This time around, the surgery should be meticulous. There is a chance of the skin being involved. Post operative facial nerve palsy is a risk to be kept in mind. The histopathologyl examination of the specimen removed should decide further management.
