Suggest Treatment For Red Circular Spots On Foreskin And Glans Penis
Question: Hello, I have a concern about some small pink / red circular spots located on the foreskin and glans. I first noticed them about 10 days ago, and they have not changed in appearance significantly since, except after the application of 1% hydrocortisone cream. This causes them to become lighter in color, and appear smaller.
The spots are most obvious after showering, which I have been doing frequently. In the two days prior to noticing the spots first appear, I did not shower, which may be significant based on my research.
After 3 days of hydrocortisone application I thought they were gone, as the ones on the foreskin were barely visible. I stopped using the cream and they returned to their original appearance. I have since purchased clotrimazole cream and have been applying it in alternation with the hydrocortisone cream.
Please let me know what you think these spots may be.
I should also mention that the spots do not itch, hurt, or have any pain whatsoever.

Brief Answer:
Avoid self medication.
Detailed Answer:
Hello and welcome to HCM.
As an Urologist, i should assure you, that don't worry about this.
Most likely, this appears to be an allergic type of rash, on the photographs.
The self medication you started with, is a steroid cream.
Avoid using it more than a week. Also, avoid using too much soap in the area.
If you've had intercourse with different partners, it's best to get your blood VDRL, HbsAg, HIV and sugar done.
Show an Urologist or a Dermatologist, if it persists beyond one week, after stopping
the treatment, with the reports.
You're welcome to clear any other doubts with me.
Wish you well.

Hi Dr. Mangat,
Should I stop using both the Clotrimazole as well as the Hydrocortisone?
As an added piece of information, it first appeared about a week after an oral sex encounter with a new partner.
It also appeared to be aggravated by a recent sexual encounter, which lead to increased redness.
In your professional opinion, is it reasonable to assume that this is not an STI / STD? I would like to rule out herpes specifically. Could i possibly be one of the rare cases where it is painless? I have read conflicting reports online.
It worries me when it becomes more red / when new spots emerge. initially there was just the 3 spots, two on foreskin and the one on glans, and then the two smaller spots on the foreskin appeared.
I will follow your advice in regards to visiting a dermatologist.
Also if this post will ever be visible to others searching online, I would prefer to have my name removed from your first reply if possible.

Hello.
While I appreciate your advice, I asked several questions in my previous reply that were not answered. I would appreciate more specific feedback.
Most importantly, I would like to know whether herpes can be ruled out in your opinion.
As a reminder, I have not experienced any pain, discharge, significant change in color or size of the spots, itching, headaches / fever.
Does your previous diagnosis of allergic reaction still stand? If not, what other possible causes of this kind of irritation / rash are there?

Allergy and STD are probable cause.
