Hi,

You need tests to identify the organism which is causing it. It may be viral or bacterial as antifungal are not working. There is possibility that it might still be fungal infection but the specific anitfungal you are using may be not covering the particular fungus.

You need KOH mount test, and some viral serological tests along with keen physical exam for deciding specific treatment.
