Suggest Treatment For Red Spot On Side Of Glans

Question: Hi there
I needed your advice on a red spot on one side of my glans.
I used hydrocortisone acetate cream for three days and the red spot almost diminished. But as soon as I stopped applying the cream, it re-appeared. The redness appears to be growing in size.

I did have unsafe sex six months ago with my fiance. Can that be a reason? Should I get tested? Though am sure about my girl.
Will an unsafe sex with this condition, transmit whatever ailment it is to my partner?

Very mild itching is there below that region where the glans end. But not a regular 24x7 one and I guess, it is not because of the redness but perhaps something else. But you would advise me better on this.
I do wash my glans with bathing soap. Can that be the cause of the redness?

And it is not a peeled patch or papule. It is redness from within. Please check my photos on this website. I have attached a few shots of my glans for your observation.

What should I do?
Resume the use of Hydrocortisone acetate? But what is the reason of the redness?
Should I see a urologist or a dermatologist?

Please advice.

Candidal balanoposthitis.

Detailed Answer:
Hi,

I read history and looked at photos you posted. Most probably you may be having candidal balanoposthitis. It is a fungal infection caused by yeast known as candida albicans. It may be transmitted by sexual contacts.

You cosult dermatologist immediately. Have blood tests to rule out DM,HIV or other internal diseases.

You may apply antifungal cream like clotrimazole cream. And itraconazole or ketoconazole may be taken for few weeks. If there is itching, take antihistaminics like levocetirizine.

Avoid to apply steroid cream like hydrocortisone you applied,that would cause flare up of candidal infection after temporary remmission.

Avoid soap bath and excessive wetness. Keep the area dry. Have Sitz bath in dettol water for cleansinfpg the area.

I hope you may be alright after few weeks.

Your partner may be treated accordingly.

I saw the reports you posted.

Reports are within normal reports. So, do not worry. As I said, the most probable diagnosis may be candidal balanoposthitis. And you may take treatment as I suggested previously, but under supervision of dermatologist. I hope you got my answer.

Hello doctor
I met a dermatologist. Since the XXXXXXX MD was not available, a junior doctor observed the condition.
She advised me a cream and a medicine, but asked me to get herpes test also done.
I have attached HSV 1 and 2 igg and igm report as well.
Also, attached the doctor's prescription for your view.
To be honest, I wasn't satisfied with the doctor as she couldn't really spell out the ailment to me.
She asked me if I have taken any medicines recently. I told her about a disprin I took a month back. I added that this isn't the first time I have taken this medicine. Though I am not too much into medicines.

Please review and let me know.

I saw report of blood. HSV 1 & 2 IgG ,IgM ..negative. So, I do not think that you have herpes progenitalis.

Clinically I feel that you may be having candidal balanoposthitis. You may take treatment as I suggested in previous post after consulting other eminent dermatologist.
