i am dr.kakkar(dermatologist). I have gone through your query and i have reviewed the image.
This looks like candidal balanitis. This is a yeast infection.
Does nt look like herpes.
I suggest that he uses a topical antifungal cream e.g clotrimazole 1%cream twice daily for a week.
In addition i would also suggest single dose of an oral antifungal e.g fluconzole 150mg tablet.

He is using cortizone 10 would that help him as well? The flucondozle i willbget him tomorrow. Also how did get a yeast infection ? If he has sex with a girl that has ot does he get it?

I suggested clotrimazole 1% cream. It is an otc product and you should be able to find it.
Yes, in addition to clotrimazole cream, fluconazole would work fine for his candidal balanitis.
It is possible that he acquired candidal / yeast infection due to sexual contact. However, candidal balanitis is is not an STD and it can sometimes arise 'de novo'
