![default](/r/images/default.png)
got the test of CBC and VDRL done which were negative.
have atached the doctors prescription , attaching the pictures also of the current state.
need advise on the current state.
Clinically looks like genital herpes; kindly provide more details
Detailed Answer:
Hello. Thank you for writing to us
I have taken note of your query and I have also gone through the attachments.
In the images I can see that there are multiple, discrete as well as confluent, well defined erosions with dirty base on the glans of penis.
In one of the images the erosions seem to be active/ fresh and in another image they seem to be healing but still there..
Clinically I suggest a possibility of Genital Herpes.
After going through the prescription of your doctor I can make out that you have NOT YET been treated for Genital Herpes though you have been TREATED for gonorrhea.
I would like to gather more information from you in order to be able to guide you better.
-Are you sexually active?if yes, with single or multiple sexual partners?
-When did you first notice these genital lesions/ erosions?
-When was your last sexual contact? Was it protected? or unprotected?
-Do you have any other symptoms like burning on passing urine, urethral discharge etc?
-Did you doctor order some lab test apart from cbc and vdrl? Did he order a test for Genital Herpes?
Kindly get back to me with these details...
Regards

Detailed Answer:
Hi.
Thank you for providing more details.
Clinically the lesions look very much like Genital Herpes.
Genital herpes is an STD and is acquired by close/ intimate contact with an infected sexual partner.
Genital herpes is caused by a virus, herpes simplex virus/ HSV. There are 2 types of HSV, type 1 and type 2. Genital herpes is usually caused by HSV type 2
The virus causing Genital herpes once acquired stays inside the body forever.
Since your recent sexual act was a protected one with your wife therefore most likely it is a past infection which was dormant and recently it re-activated to form sores.
Genital herpes once acquired may stay dormant in the body and can re-activate from time to time to give rise genital sores.
Only a few genital STD's present with genital sores/ erosions (i.e Genital Herpes, Syphilis, and Chancroid) and the lesions on your glans look very much like genital herpes.
I would request you to kindly get screened for genital herpes as well. You may take up an ELISA TEST for type 1 & type 2 HSV (IgG and IgM).
Though your treating doctor did keep a possibility of genital herpes but he did'nt order a test for genital herpes..
I would suggest that you request your doctor for the same.
This discussion is meant for patient guidance and not for prescription purposes. Therefore I cannot write you a prescription online.
Kindly also go through the disclaimer at the bottom of this page.
You can get back to me for any more clarifications.

Oral antivirals like acyclovir Or valayclovir
Detailed Answer:
Hi.
Antivirals like Acyclovir Or Valacyclovir are effective for genital herpes. The usual treatment duration is 5-7 days for an episode of genital herpes.
Genital herpes may recur and subsequent episodes can be treated in exactly the same way as original episode.
Some patients who have very frequent recurrences of genital herpes may require continuous suppressive treatment with oral antivirals for 6-12 months.
Kindly contact your doctor for dosage and prescription.
Topically you may use an OTC antibacterial e.g 2% mupirocin ointment, twice daily, only as long as the sores are present.
